Gem Home Care is your best source for basic support services. We provide services to all ages who need basic services at home. We provide a unique community-based service that assists our clients and allows them to remain safe and independent in their own homes.
Here are some of the services we can provide:
Personal Support
Our Support is geared to give unique assistance to clients. Our Support is a traditional service that is available to an individual on the developmental disabilities waiver. Personal Support service is provided at the client’s home. Personal Support assistance with daily living activities, such as bathing, eating, dressing, toileting, meal predation, grocery shopping, medication management, and light housekeeping.
Homemaker
Our Homemaking service shall take care of all your household chores like cleaning the house, washing the dishes, washing the clothes, organizing the closets, cooking food, and caring for the garden and pet. This service is best for those who can still perform personal tasks but need assistance with home management.
Adult Companion Services
Through our Adult Companion Services, elderlies no longer need to spend most of their time alone as our caregivers provide them with a confidant to whom they can talk, a chaperon when they need to be out, a hand when they need assistance, and a friend whom they can be with as they wish. This service is best for those who are away from loved ones and need someone else to maintain their emotional and mental wellness.
Respite Care, In-Home or Out-of-Home
Our Respite Care is a short-term care service provided due to the absence or need for relief of the primary caregiver normally providing the care. Respite Care can be used for daily living, such as grocery shopping; emotional support through attending a support group such as church service or mosque service; socialization by spending time with friends or family outside their home environment; or self-care, such as a doctor’s appointment or simply staying home with the client while the primary caregivers are away.
24-Hour Emergency Assistance
Our 24-hour emergency assistance is available to clients who live in their homes does not receive 24-hours supervision or otherwise require extensive, routine supervision. The services we will provide are an immediate response for assistance at a client’s home, problem-solving, and on-call counseling.
Night Supervision
Night Supervision we provide is overnight assistance and monitoring in the home of a client for a period of no more than 12 hours to 24 hours. Night supervision carries out behavior programming and plans as they relate to the living skills and interventions of the person and include the following:
reinforcing independent living skills
assisting with incidental daily activities
